In their letter, Chirumbolo et al. discuss the fact that most of the adverse effects of OOT (Oxygen-Ozone Therapy) are consequences of malpractice and absence of protocols [1]. Unfortunately, we do not have enough information to exclude this possibility, as the OOT protocol was not available at the site where it was performed, and there was no way to claim it because the patient and his family did not pursue any legal actions. However, assuming malpractice as the potential cause of injecting a large volume of gas into a vessel, it cannot entirely explain the extensive compromise seen in this patient, with bilateral, anterior, and posterior infarctions. The only way to explain this pattern of stroke is through multiple dissections of both vertebral and carotid arteries or venous embolism leading to cardioembolism. However, both possibilities, as well as other etiologies, were properly excluded through an extensive work-up. …
