The reported rate of symptomatic internal carotid artery (ICA) occlusions is approximately 6/100,000 individuals [1, 2]. The natural history and optimal management of a chronically occluded ICA (COICA) are controversial and there is insufficient data concerning endovascular revascularization [3]. Nonetheless, the annual stroke risk in these patients ranges from 2% to 7% under medicinal therapy alone [2‐5] and stroke etiology is related to impaired hemodynamics and/or embolic phenomena [4]. …
