10. Interventionelle Sonografie der Hand

Zusammenfassung

Sonografisch gesteuerte Interventionen sind wesentliche Ergänzung in Diagnostik und Therapie der Hand. Die visuell kontrollierte Applikation von Medikamenten kann deren Wirkung erhöhen und den Infiltrationsschmerzen lindern. Zielgenau applizierte Lokalanästhetika können den Effekt einer operativen Therapie simulieren und die Differenzierung zwischen anatomisch eng beieinander liegenden Schmerzursachen ermöglichen. Durch therapeutisch applizierte Medikamente können operative Therapien hinausgezögert oder überflüssig werden. Sonografisch gesteuerte therapeutische Interventionen können in ausgewählten Fällen eine Operation ersetzen. Die rechtlichen Aspekte und Hygienemaßnahmen sonografisch gesteuerter Interventionen sowie Wirkmechanismen der zur Applikation zur Verfügung stehenden Medikamente werden in diesem Kapitel detailliert behandelt. Es werden Indikationen und Studienergebnisse diskutiert und der Einsatz der Methode anhand der häufigsten Indikationen umfassend erläutert.

Das Karpaltunnelsyndrom ist die häufigste Kompressionsneuropathie peripherer Nerven. Obwohl die Anamnese mit dem nächtlichen Einschlafen der Hand (Brachialgia parästhetica nocturna) sehr typisch ist, ist eine klinisch-neurologische Untersuchung und Elektroneurografie in manchen Fällen auch eine Neurosonografie erforderlich. Im Anfangsstadium sind konservative Maßnahmen (Handgelenksschiene, Ergotherapie) empfehlenswert. Bei nicht Ansprechen der konservativen Therapie oder Auftreten von neurologischen Ausfällen ist eine Dekompression des N. medianus am Karpaltunnel indiziert.
