In the commentary by Song and Bretthauer regarding the competing mechanisms of action involved with screening colonoscopy, the authors call for more methodological research to quantify the relative contributions of colorectal cancer (CRC) prevention versus CRC early detection [
1]. In 2019, my colleagues and I published such an analysis of flexible sigmoidoscopy (FS) and fecal occult blood test (FOBT) clinical trials which indicates that CRC prevention appears to be the major mechanism for reducing all-cause mortality in those trials [
