Autologous fat contains mesenchymal stem cells, with anti-inflammatory, angiogenic, and immunomodulatory properties allowing tissue repair. As a result of these properties, autologous fat injection (AFI) has been tested in the treatment of both cryptoglandular and Crohn’s disease anal fistula. In cryptoglandular fistula, the addition of AFI to the closure of the internal openings did not show any benefit in terms of fistula complete healing rate [
1]. However in patients with perianal fistulizing Crohn’s disease, AFI is safe [
2], and associated with a clinical response (resolution of symptoms) rate of 76% and a complete healing (resolution of symptoms and closure of fistula openings assessed by clinical exam) rate of 57% at 6 months [
3]. Complications related to AFI include perianal abscess associated with the injection of high volumes of fat in 9.5% of patients, proctalgias, and abdominal hematomas in less than 5% of patients [
