Zusammenfassung

Die Therapie des diabetischen Fußes kann nur interdisziplinär und fachübergreifend gelingen. Im Rahmen der konservativen Therapie stellt das TIME-Konzept ein erfolgreiches Modell dar. Neben der konservativen Lokaltherapie und Wundbehandlung und der sachgerechten Infektionstherapie kommt der Optimierung der Gefäßsituation und somit der Sauerstoff- und Nährstoffversorgung im Wundareal ein besonderer Stellenwert zu. Die Gefäßrekonstruktion erfolgt idealerweise minimal-invasiv radiologisch interventionell oder bei ausgeprägten Befunden durch entsprechende gefäßchirurgische Maßnahmen. Bei Notwendigkeit einer Amputation sollte eine Minor- (unterhalb des Knöchels) oder auch Syme-Amputation angestrebt werden, um eine möglichst große Mobilität des Patienten zu erhalten. Sofern nötig müssen selbstverständlich auch Major-Amputationen durchgeführt werden. Bei größeren und komplizierten Gewebsdefekten ist zusätzlich die Expertise des plastisch-rekonstruktiv tätigen Chirurgen gefragt.
